Publish Forms in 2007

Turn on the Developer tab, using the instructions given at
http://www.outlookcode.com/article.aspx?ID=35 . Once you do that, the Publish

You don't need (and shouldn't bother with) a distribution list for that.

That's not possible without writing code to create the item and then copy
data into the individual fields. And to do that, you'd have to know exactly
how MS CRM stores its data.
