Default Inserting pdf file as icon - cannot get picture to show up

I don't think I've ever had the PDF show up as a true thumbnail in this
situation -- it's always just the PDF icon.

What is it you need, exactly? Is it just to display the PDF during slide
show view? You can do that with an animation setting. Select the PDF icon
and on the Animation Pane, choose Add Effect. Select Object Action | Open
and specify that it start automatically, which will open the PDF when you
come to that slide.
