Perhaps something like:

SELECT * FROM tblAddresses WHERE PatientID = RequiredPatientID AND
AddressFromIndex = RequiredDateIndex AND AddressToIndex =
RequiredDateIndex

Ohmigod I didn't say SELECT * did I?
