Default Making a Word template based on an existing paper note

I am trying to reproduce a form that we have and make it into a Word
template. This form resembles notebook paper in that it has established
lines to write on. I would like to create a Word template so that a person
could fill out this form on the computer. I have tried to do this, however
when I type on the lines, instead of the sentence wrapping around to the next
line it just increases the space between the lines. Does this make sense?
Can you help? Thanks...
