Default Check Boxes

I am creating a new database where a user will have to answer each question
with Yes, No or N/A. I want to use a checkbox so that I can get a total of
how many questions were answered yes, how many were answered no and how many
were answered N/A. What type of checkbox should I use. Should I use a combo
box?

IMO a combo box or an option group would better suited to your needs.

Default Check Boxes

Use comboBox or Option group. Personally for UI sake if the other stuff is
check boxes then I would use an option group with the three types to keep the
look consistent.
