Default How do I change the fields available in a new contact?

I am trying to set up my outlook so that it will accomodate my church
directory. I would like to have the fields that come up to fill out be the
same as the fields that I have customized in my view. How do I do that
without having fields that are never going to be used showing up in the
contact information?
