Default How do I create a reader service db?

I have a magazine that has reader service cards and I want to collect the
data from those cards and send respondents via email, fax, or label the info
on each advertizer that they are interested or picked. Example Fall issue RS
#1 is Bound Tree, but that number will change in the Dec. issue and #1 will
be Wells Cargo how do I seperate the two? How do I set it up to print, fax or
email 50 or different faxes and emails? Thanks

This newsgroup (tablesdbdesign) focuses on answering fairly specific
questions about tables in MS Access. If your question is about using
Access, it may be broader than the focus of this group.

(or some other reader may be able to point you directly to an answer!)
