Default 2000 Journal Entries assigned to incorrect contact names

Over the years, Outlook 2000 has been updated, migrated from one computer to another, repaired and reinstalled on different operating system.
Journal entries made several years ago under one contact, say X, now show up under the journal tab for another contact, say Y.
Is there any way to reassign the journal entries (phone calls, emails, tasks) from Y back to the correct contact (X)?
I have tried the 'Detect & Repair' option...did not work.
Tried to physically drag and drop the journal entry...will not work.
Help....please assume I am not very proficient with Outlook and database terminology or very computer savvy.
