Can't figure things out with Form making in Word

Default Can't figure things out with Form making in Word

For the life of me, this senior citizen has problems with using Word and
making forms. Is there a program that is easier, and then allows me to save
it as a Word document. I can't figure out how to make a form header, like
"The Company" then get out of that mode and make boxes with writing inside
of it, like "from:" and "to:" I have FormFlow and JetForm, and liked them a
lot, but they are not in business anymore, and as far as I know, can't save
into Word? Any help is appreciated...........Peter

Many Thanks Doug......This is the best example I've seen so far, as to how
I should do it........BTW, a senior citizen to me is a person that can get
Social Security. I'm sure some people will disagree, but it is also how one
feels. I feel like I'm 105 all the time, but I'm trying............Peter
