I am using a combination of Ask and Ref fields in a document and would like
to have all the Ref fields display a value of "N/A" when the document is
first opened.

In essence, the way to do that is to ensure that every bookmark referenced
by your REF fields has the value "N/A" when you /save/ the document.

Because each REF field references a bookmark that is set up by
a. bookmarking some text in the document
b. a SET field
c. an ASK field
that means that you may have to fix a number of different things to achieve
what you want. However, it's probably just a question of ensuring that you
have been through all the ASK questions and provided "N/A" (without the
quotes) as the answer, then saved and closed the document. Then you need to
make sure that the users cannot change that starting document - e.g. make
sure it is read only.

There may well be more to it than that, but that's the starting point.
