Default Zip Code Tables

I would like to be able to enter a zip code into my form
and have the city and state automatically fill in from a
table. Where can you go to import zip code data?

If you are looking for something free, but a little old, try

http://www.census.gov/geo/www/tiger/zip1999.html

but you will need to join with the states via the fips codes that may be
here
http://www.census.gov/geo/www/fips/fips.html
