Default Business Contacts Corrupt

The next thing to try is to delete the BCM forms completely and let the COm
add-in republish them. Use Tools | Options | Other | Advanced Options |
Custom Forms | Manage Forms. Display the forms in the Personal Forms library
and delete the Account and Business Contact forms. Then shut down Outlook,
wait a few minutes (to make sure it shuts down completely, and then restart
Outlook.
