Default Switchboard

I created a switchboard and for some reason it does not appear in my forms.
I tried to delete it but it says that you cannot delete the default
switchboard. How can I redo it?

Default Switchboard

A switchboard has 3 parts to look at. 1. The form itself. 2. It creates a
table with your switchboard entries so that it can save them. 3. A startup
under tools that tells what form to start when the database starts up. Then
under tools there is a switchboard manager. One of those should be the
problem.
