Default "Microsoft Outlook has encountered a problem and needs to close...

I get this error message literally every 2-3 minutes. Sometimes after I
close the message it crashes Windows as well - I get a blue screen and memory
dump. I'm on Windwos XP w/ SP2 and using Outlook 2003. I've installed no
new programs recently. Problem just started about 5 days ago. I tried
"detect and repair," installed outlook SP1, ran scanpst on my .pst file, ran
symantic antivirus and Ad-Aware, tried outlook in safe mode, nothing is
working. It's extremely discouraging - any help is MUCH appreciated.
I use outlook for two pop accounts and an exchange account.
Below is the error message details:
