Default Template for state of Alabama Certificate of Immunization

We fill out many, many certificates of immunization for the children we
vaccinate at our office. It would be nice to have a template where we type in
the dates the immunizations are given, add to it, and retrieve and print the
document on the state approved "Blue Card" for the children of Alabama. The
document is known as the State of Alabama Certificate of Immunizaiton.
