Default Questionaire

How do I make up a questionaire that would ask additional questions based on
answers to previous questions?
ie: Did you hurt your upper body or lower body? Ans: upper Body
The next question to pop up should be "what part of the upper body" then
have a list of upper body areas. If the ans was lower body then the question
to pop up should be "what part of the lower body" and so on. All questions
would have a list to choose from.

I would probably modify At Your Sruvey found at
http://www.rogersaccesslibrary.com/f...osts.asp?TID=3. You could add
another field to the table of questions that directs to another question
based on the response.
