Outlook custom contact form

Default Outlook custom contact form

Hello, I setup an outlook custom form on an exchange server early last year
and it worked fine for a long time, 2 weeks ago one user told me they were
no longer able to add a contact and have it use the custom form, it defaults
back to the default IPM.Contacts, and not IPM.EmailForm06. it only happens
on this one PC, and im not sure where to start looking for the problem.. I
have thought about removing her outlook profile form the PC and resetting it
back up, but I havn't had the time to do that and was wondering if there was
something else i should try first, or if that didn't fix it.

Have you tried clearing her forms cache?

More info: the error she gets is this:
"The custom form could not be opened.. Outlook will use an Outlook form
instead"

but it works on everyone elses' PC

i had her look for frmcache.dat and she said she could not find it.

Forms normally is a hidden folder. She can use the interface built into Outlook's Forms Manager dialog.
