Mail Merge Problems--Word 2000 with Outlook 2000

Default Mail Merge Problems--Word 2000 with Outlook 2000

Have all my address records into Outlook 2000. I go
through Tools/Mail Merge in Word...which allows me to
choose the "Outlook Address Book".....I go through the
setting up of the mailing label and "Insert" what fields I
desire into the sample label.
When it merges, however, ONLY the Title, first, and last
names show up...no street address, city, state, or postal
code.
Sure can't seem to find out what I'm doing wrong.
Any help would be GREATLY appreciated!!!
