Default Custom fields on default Contacts form

Is it possible to have user-defined fields appear on the default Contacts
form? If so, how is this done. I'm not having any luck creating
user-defined fields. It seems that I have to create an entirely new form.

Default Custom fields on default Contacts form

You need to design a new contact form with the desired fields. Then publish
it and make it the default form. See

FYI, there is a newsgroup specifically for Outlook forms issues "down the
