Custom fields on default Contacts form
Is it possible to have user-defined fields appear on the default Contacts
form? If so, how is this done. I'm not having any luck creating user-defined fields. It seems that I have to create an entirely new form. Thanks.

Custom fields on default Contacts form
You need to design a new contact form with the desired fields. Then publish
it and make it the default form. See http://www.outlookcode.com/article.aspx?ID=39 for details.
