Default Duplicate meetings being generated (Outlook 2003, Exchange 2003)

One of our clients is having an issue that when Bob organizes a meeting on
behalf of Jane, Steve receives two calendar requests - one from "Bob on
behalf of Jane" and the second is simply from Jane.

I'm sure there is a fairly straightforward solution to this one so that only
one request is generated, the one from "Jane"
