Default "At Your Survey" Design Question

All,

Thank you very much for your suggestion to check out the "At Your Survey"
database - this proves very helpful.

I did have a question regarding its design and its pertenance within my
project. Within my project I will have patients that will be taking multiple
surveys. I would like the Patients to be the top hierarchy on my form, and
below would be the surveys and the survey description. What would be the
best way to structure forms based on the AYS design and would there be a need
to add any additional tables for the many-to-many patient/survey relationship?

You could add a field to the table tblSrvRspns. The field could correspond
with a PatientID.
