Default Trying to break up data in the label format to fields, for easy entry into table

Hello!
Say:
If I have the following data in a word document

Ms. Johanna Sullivan
Sylvan Technology Center
3810 Valley Center Dr., Suite 904
San Diego, CA 92130
858-481-3640

Ms. Sara Taylor
Texas Instruments
6412 Hwy 75 S., m/s 816
Sherman, TX 75091
903-868-6168
s-tylor1@ti,com

Timothy M. Brown
Ballotti Learning Center
Suffolk University
41 Temple Street
Boston, MA 02114
617-573-8235



is there any way I could break the data into fields say
F_name, L_name, .......... Phone, e-mail; and then create
a database with those fields?

Like I already tried to work with the mail merge helper
in the word,to create a new data source, but I don't have
that specific button,(in the mail merge toolbar).

Will be great if you could help me with this, or even if
some one could write to me how to download that
particular button.
