Default Adding Children to Details Tab

I have a 2 part question.

1. How do I use the form designer to add a field on the "Details" tab of a
contact so that I can added the field "Children"?

2. How do I apply this to all of my contacts in Outlook 2003?

1) You can't. That page is not customizable. Use a different page.

How would I publish the new form to the public folders on my exchange 2003
server?
