Default Wait Time Report

The 'wait time report' is a MS Access Form that enables/provides all access reports (scroll-down) and select the report to process. At the same time it provides to/from date selections, copies etc.

Default Wait Time Report

I'm interested in the 'wait time report' access form to include in my applications so the operator can select the report using the scroll down method. I have seen this form before and it's a great tool for all access applications. Is this form available?
Thanks, Don
