Default Is there a template for documenting a patch panel?

I'm a Information Systems Site Manager and I'm moving to a new building, I'm
trying to record all the connections on my new Patch panel and was hoping to
use either Excel or Visio to record all the connections on the Patch panel to
the corresponding offices.
